
If you have been impacted by an unforeseen disaster or personal hardship, you may be eligible to apply for financial assistance.

The Associate Assistance program is managed by the Emergency Assistance Foundation (EAF). EAF independently administers all application activities.
Who can apply for a grant?
Eligible individuals include:

Associates actively employed by Ascension Wisconsin for more than 90 days
-
Full time, part time or PRN (working at least 24 hours a week)
-
On an approved leave of absence for no more than six months

When can I apply for assistance?
Grant applications may be submitted when you have been impacted by one of the following unforeseen events and one or more of the associated expenses:​

Natural disasters such as flood, wildfire, tornado, earthquake, blizzard, severe storms, etc.

Terrorist actions
-
Disaster resulting from a vehicular accident

Military deployment
-
Civil unrest
-
Impacts to primary residence such as fire, flood or unusual life-altering expense not covered by insurance
-
Serious illness or injury
-
Non-routine/exceptional medical expense
-
Victim of a violent crime
-
Domestic abuse
-
Death

What are the criteria to apply?
Qualification criteria includes:

You or an eligible dependent were impacted by one of the above-mentioned scenarios
-
Application submitted within 180 days of the qualifying event
-
If a previous application was not rewarded, you must wait 30 days to apply again
-
IRS regulations require proof of the event that has impacted you or an eligible dependent, as well as proof of the resulting expense(s) for which you are seeking assistance
-
Supporting documentation must be dated within 60 days of the application date

How much assistance can I apply for?
The award maximum is $1,000, and the minimum amount that may be requested is $500. The amount awarded is based on individual financial need as demonstrated through required documentation.

A lifetime, maximum amount of $4,000 may be awarded.
